Overview
What is it?
The community care services that you could get will vary depending on the Health and Social Services Trust area that you live in. Since April 2007 there are five Trusts in Northern Ireland, and their websites are listed in the useful links section. You can also visit the new ‘gateway’ site at

Community care services could include home help services, day centres, and access to more specialist support such as for continence or dementia care.
How do I get it?
For more information about this you can contact a local advice service.
To access any of the services provided by your local trust, you should ask your GP or social worker to refer you for assessment. You will probably then be visited by an Occupational Therapist who will discuss your needs with you.
